The incident occurred on July 21, 2025, at Lake Arrowhead in Southern California. According to early reports, Warner was swimming with close relatives when he disappeared below the water’s surface and could not be immediately located.
Emergency crews responded to a 911 call placed around 3:45 p.m. local time. Despite exhaustive rescue efforts, Warner’s body was recovered later that evening. He was 54 years old.
